FormFire is an online, interactive interview that collects information that is stored securely and is HIPAA compliant. The information can be submitted to a number of health insurance companies on this private and secure system, with no software to install or maintain.
We will send you a welcome email containing an introduction to FormFire as well as all instructions necessary for you to get up and running. This welcome letter contains the necessary login information for each employee, with instructions on how they can create their accounts.
Your employees are guided through the dynamic Employee Interview and Medical Health Questionnaire, ensuring all relevant data is collected.
At the end of the interview, the employee must electronically sign his or her application in order to “complete” the process.
